Trust Formation Documents

Under the Know Your Customer provisions of the Patriot Act of 2001, FCMs and other financial institutions are required to obtain proof of identity in conjunction with the account opening process. Depending on the type of account, country of origin, citizenship and other factors, you may be required to submit additional documents along with your completed and signed account forms.
